Fitness and Nutrition Statement

In consideration of being permitted to participate in fitness and nutrition classes and demonstrations with The Foundation for Black Women's Wellness (FFBWW), I agree to assume full responsibility for any risks, injuries, damages known or unknown which I might incur as a result of participation. In further consideration of being permitted to participate in fitness and nutrition activities and education offered by FFBWW and its partners, I knowingly, voluntarily and expressly waive any claim I may have against The Foundation for Black Women’s Wellness or other fitness or nutrition professionals they retain for injury and damages that I may sustain as a result of participation. I, my heirs or legal representation forever releases, waive, discharge and covenant not to sue The Foundation for Black Women’s Wellness for any injury or death.

Media Statement

In consideration of participating in events with The Foundation for Black Women's Wellness event, I agree to being photographed, filmed, and/or otherwise recorded. My attendance at the event constitutes consent to such photography, filming, and/or recording and to any use, in any and all media in perpetuity, of my appearance, voice, and name as captured for any purpose whatsoever in connection with the organization and production presently entitled: the Foundation for Black Women’s Wellness.
