In consideration of being permitted to participate in fitness and nutrition classes and demonstrations with FFBWW, I agree to assume full responsibility for any risks, injuries, damages known or unknown which I might incur as a result of participation. In further consideration of being permitted to participate in fitness and nutrition activities and education offered by FFBWW and its partners, I knowingly, voluntarily and expressly waive any claim I may have against The Foundation for Black Women’s Wellness or other fitness or nutrition professionals they retain for injury and damages that I may sustain as a result of participation. I, my heirs or legal representation forever releases, waive, discharge and covenant not to sue The Foundation for Black Women’s Wellness for any injury or death.
